When do you need an apostille?
Businesses that want to do business outside of the country must obtain the Corporate documents apostilled for use in that respective country. Certificate of Insurance, Incorporation Documents, Annual List, Power of Attorney are some of the documents that have to be apostilled for use in Italy.
Individuals that have interests outside of the country or were born in the USA and move to another country will need documents to be apostilled. Individual and child's Birth Certificates, College Transcripts, Professional Accreditation, Passports are some of the documents that have to be apostilled for use in let's say Italy.
Apostilles can only be done for use in Countries that are part of The Hague Convention. list of participating countries below:

This process always takes longer, sometimes 4-6 weeks and sometimes 5 months or more (like China).

All documents should have original signatures, original notary signatures with a raised seal and corporate documents should be certified or issued by the Secretary of State.
The original document to be apostille is received by us. Whether by mail or by hand. That document should be originally signed and notarized. In some States, the document has to be submitted with the County Clerk's office and they authenticate the notary and notary signature (document is flagged). Some states don't require this process. The document once flagged (or if not needed by the State), is submitted with the Dept of State. The Dept of State then adds their seal and a cover page with a ribbon or a flag as the front page of that document. That document is now apostilled. Once the document is sealed (stapled of some sort) the pages must not be separated or altered.
For counties outside of The Hague, all the above has to be done, then the document has to be submitted with the US Dept of State. The US Dept of State will add in their special seal or flagging. Once this is completed, the document is submitted with the respective Embassy (China, Taiwan, UAE etc).
Most Powers of Attorney, Affirmations/Affidavits, Letters etc. to be apostilled must also contain a translation in the language of the country where the document is to be used.
